A streamlined referral process for clinics, healthcare providers, and partner organizations.
Use this secure form to refer a client to Kindbridge Behavioral Health for evaluation and treatment. After the referral is submitted, the client (or their guardian) will receive an appointment link to schedule their initial engagement session at their convenience through our self-scheduling portal. If no appointment is scheduled, our engagement team will follow up within 1–2 business days to provide support and discuss available services. With client consent, we will keep you informed to support coordinated, continuous care.
